North Carolina is unusual in how much of its complex care is concentrated in a handful of academic centers, with excellent community hospitals handling everything else. That geography shapes what happens to patients here. A serious diagnosis made in a community hospital often means a referral to a tertiary center an hour or more away, a new set of specialists who have not spoken to the first set, and a family suddenly coordinating two institutions that do not share a chart.
That handoff is where cases go wrong. Imaging gets repeated because the disc never arrived. A pathology slide sits unreviewed at the second institution for a week. Someone assumes the referring physician is still following the case, and nobody is.
I trained inside this system. I know how records actually move between North Carolina institutions, what a referral queue looks like from the inside, and which questions cause a scheduler to find an earlier slot. When a case requires a center outside the state, I can tell you honestly whether the trip is likely to change the plan.
